Wheelchair license Plate: Drop Box Instructions

Drop boxes are located at all of our service centers (Access to our drop boxes varies by location, for more information visit our Service Center Locations).

Please submit the following items in a sealed envelope and place it in one of our drop box locations:

1. Completed form HSMV 83007, Application for International Wheelchair License Plate, or form HSMV 83039, Application for Disabled Parking Permit.
2. A copy of the vehicle registration certificate.
3. Copy of Florida driver license or ID card.
4. Proof of Florida insurance for the vehicle.
